Question - how should I go about putting together a basic form/database 
application?


Background - I run a small organization with a few hundred volunteers. 
We've got piles of handwritten signup sheets, contact info, insurance 
forms, that sort of stuff.  What I'd like to do is set up a little 
self-serve station where people can enter and check their own info so 
that we don't need to go blind and nuts transcribing.  Nothing terribly 
interesting.

My first thought is to put together some type of LAMP package on a 
standalone box; we don't have Internet access.  Recommendations?
