I had the same card for a while, it's an Audiovox PC5220, though I'm running a 
Kyocera KPC650 now.  Take a look here for step-by-step instructions on getting 
it working:

You may need to do a bit of tweaking, but the ka9q site walks you through it. 
The only thing you may need to do with Windohs is update the card's firmware 
and coverage map.
