Pure speculation: The CentOS grub entry was pre-repartitioning, and
now refers to a differently numbered partition?  In which case, it's
"mostly right" but one or more of those (hd0,#) entries has to change.
Or possibly some weirdness involving Logical Volume Management (LVM),
which I've done battle with in the past.  (I noticed in a prior
message I saw LVM in one of your command outputs.)
