Under LVM-over top of-softRAID?  Three or four times during the whole
time.  I don't have hot-swap capability at home or at work so I had to
power the machine down, crack the case, and swap the bad drive out.
When the machine was brought back online the replacement had to be
partitioned to match the remaining drive.  Each partition was re-added
to the existing RAID, but once the drives synched back up everything was
right as rain.
