SAC 21.4 - Don't close already-closed file descriptors

SUPERCEDES APPROVE COMMIT 21.4

This patch has been committed to 21.4.  It removes the code that
closes the already-closed file descriptors and also slightly cleans up
checking for executable attribute under Windows.
